ATHLETE REQUIREMENT INFORMATION

      The following information is provided to explain to both students and parents the requirements for participation in athletics. Each of these items must be completed before the athlete is allowed to begin practice. Forms should be turned in to the Athletic Coordinator/Secretary, in the main office of the school, who will clear the athlete for competition by issuing a PINK CARD.

Athletic Participation Card

This form must be completed in its' entirety. Participation form is acceptable for two consecutive years if all information is current.
Any student who might be wrestling should be sure the examiner fills out the minimum weight section.

Co-Curricular Athletic Code Handbook

This for must be completed in its' entirety each new school year. The athlete's and parent's signature on the Handbook is essentially a contract whereby he/she agrees to abide by the rules and regulations set forth in the Co-Curricular Athletic Code.

Insurance Program

 

All student athletes must be covered by accident insurance. They may be covered by a family plan or by the plan offered through the Tahoma School District. School insurance forms may be picked up at the school office.

     

ASB Card

 

$30.00 Fee ($30.00 7th - 9th grade)  All students participation in athletics must be holders of an ASB Card. The Associated Student Body provides financial support for all athletic activities. High School students who are holders of the ASB Card receive free admission to all regular season home athletic events, and reduced prices for dances and away activities.

     

Sports Physical Examination

 

 

Every student is required to have a complete physical prior to participation in school athletics. This physical must be administered by a medical authority licensed to perform physical examination. The physical is good for 2 consecutive years and must be valid through the entire sports season. The following are licensed authorities:   

 

M.D.

Medical Doctor
  D.O. Doctor of Osteopathy
  C.R.N. Certified Nurse Practitioner

 

P.A.

Medics-Physician Assistant

Record of this physical must be on file with the Athletic Coordinator.

Per WIAA rule 18.13.0  Prior to each subsequent year of participation, a student shall furnish a statement, signed by a medical authority licensed to perform a physical examination, which provides clearance for continued athletic participation. Physicals dated after June 1,2004 are valid for 24 months from the date of the examination. Physical examination dates must extend beyond the respective WIAA season ending date.

     

Fines

  All school fines must be paid in full prior to clearing for a sport.
