Fall 2009-2010 Registration Information

To register for the 2009 – 2010 Extended Enrichment program, please complete and sign the two page registration form and submit along with your Annual registration fee to:

2009 - 2010 EEP Consumer Debit Authorization Form (Please complete, attach a void check and return with your registration packet if you would like your EEP payments deducted automatically.)

Registration forms will not be accepted until Monday, May 18th for current EEP families, or Monday June 1st for new incoming families.

Extended Enrichment roster positions will not be assigned if form is incomplete or registration fee is not included.
